Continuing Care
The Continuing Care program at Kaiser Permanente Northwest cares for members with a wide variety of conditions who require ongoing rehabilitation, usually after a hospital stay. Common conditions requiring a skilled rehabilitation admission are stroke, hip fracture, need for intravenous antibiotics, or complicated wound care. We partner with community skilled nursing facilities, embedding Kaiser Permanente physicians, nurse practitioners, and nurse care coordinators in multiple core sites across the region. Many of our physicians are geriatricians, and some have a background in hospital medicine. Our program is fully integrated into the Kaiser Permanente continuum of care and connects members back to primary care and home health services, if needed, upon discharge from rehabilitation.
